Instructions: Need to go there first and get registered. This process takes at least 24 hours before an appointment can be made. Bring with you your Tribal ID card (they will call Oklahoma to verify), a picture ID and documentation of any other insurance. This looks like a huge facility with many services, so it would be worth the effort.
Instructions: Must have lived in the area at least 30 days PRIOR to being seen. Must come in and fill out their new patient application. If employed, must bring two months of pay stubs as some services are provided on a sliding scale. After application process is complete, an assessment is performed to determine what level of care is required. No one is denied services based on lack of ability to pay.
The staff have stressed that you are eligible for direct care only - again, anything referred out will generate a bill to you. They have general medical, dental, optical, and their own lab, etc. It is possible to make a same day appointment once in the facilities system by calling early (8:00 am). Hours are M- F 8:00 to 4:30 except Weds. 10 to 4:30.
